Book Appointment

Appointments for registered clients can be booked by calling clinic reception at 416-593-7655 ext 7. All clients are reminded at the time of booking appointments to provide 24 hours’ notice when cancelling appointments. If you already have your first appointment, you can save time and fill out your registration form ahead of time. Please download our registration form HERE.

After Hours

Clients wishing to speak to a physician after hours may leave a message with the centre’s phone service and the on-call physician will return the call as soon as possible. In the event of an emergency, clients should report to the nearest hospital emergency room.

Test Results

Test results are generally not given over the phone. Clients are encouraged to make an appointment with their provider to privately discuss test results or review the information located in their medical records.

Online Booking

Online booking is available for current clients of the centre who have a valid Ontario Health Insurance Plan (OHIP) card.

If you are not a client of the centre, any appointment you book will be automatically cancelled.

How to Book

  • Use the booking link or scan the QR codes posted throughout the centre and on provider doors.
  • Once on the platform, select your provider and choose from the available appointment slots.
  • Only certain providers and appointment types are available for online booking. The list of active providers will be visible on the platform.

Appointment Types

  • Triage Appointments: Same-day only, available from 6 am (or the specified time) until all slots are filled. These are 15-minute appointments.
  • Follow-Up Appointments: Can be booked in advance. Only a limited number of 30-minute slots are available per provider.

Cancellation Policy

  • If you need to cancel your appointment, please provide at least 48 hours’ notice.

Accessibility & Technical Information

  • The online booking platform is only accessible to clients with OHIP.
  • If you do not have OHIP, please call our administrative staff to book your appointment.
